FREE Non-Emergency Medical Transportation through Primary Care Solutions is an important service for individuals needing assistance getting to and from health-related appointments and pharmaceutical pick-ups. Medicaid beneficiaries provide NO out of pocket costs for any of the transportation services rendered.

Primary Care Solutions NEMT is a great choice when you really need someone to take you to appointments when the need is not immediately life threatening. The next time that you need to take a trip, consider non-emergency medical  transportation. 

 

You can make this call directly or have your caseworker, caregiver, doctor’s office, therapist, a friend or relative call the number listed and ask to be transported by Primary Care Solutions. It’s that simple. Just be ready and on time for pickup for a safe, professional and efficient transport to and from your destination. 

Our transportation service is FREE to Medicaid beneficiaries.

Accessing Services


Primary Care Solutions provides a wide variety of services that are accessible to many locations throughout the state of Ohio. Visit our Adult and Childrens page to find the service that best fits your child's needs.


Completing an Assessment


Each client entering services thru Primary Care Solutions will need a Comprehensive Clinical Assessment to determine the best treatment option for their care.


There are several ways to receive an assessment:

  •  You can contact your local PCS site. The phone numbers to reach them will be on the Locations Page.

  •  If you don’t have a phone, you can come to one of our sites and get an appointment scheduled.

  •  Fill out our referral form directly on this site and one of our intake coordinators will call you to set up an appointment.


Once you have completed your assessment you will be contacted by a case manager to speak with you about your treatment plan and discuss the type of services that have been recommended by the licensed professional that completed the assessment.


After going over your treatment plan your assigned Case manager will take on the responsibility of addressing your clinical needs linking you to resources in the community. The case Manager will also help provide more in depth support as needed to troubleshoot barriers to treatment as they occur.
